Key Responsibilities:
- Manage pain and other distressing symptoms such as dyspnea (difficult or labored breathing), nausea, anxiety, and delirium.
- Provide inpatient palliative care consultations to patients with advanced or complex illnesses.
- Assist in management of end of life symptoms in the inpatient palliative unit
- Facilitate advanced care planning, including goals-of-care, advance directive, and code status conversations.
- Collaborate with interdisciplinary team members, including nurses, social workers, chaplains, and case managers.
- Coordinate care with primary teams, sub-specialists, and case managers.
- Assist with transitions of care between different settings.
- Document all care and decisions in the electronic medical record according to organizational standards.
- Educate patients, families, and hospital staff about palliative care principles and options.
- Participate in team meetings, quality improvement initiatives, and teaching.
- Involved in teaching of Internal Medicine and Family medicine residents (schedule rotations with 2nd and 3rd year residents)
